Avon Pediatrics is closing permanently on April 30th, 2026 due to Dr. Hensley Retiring.

To request a copy of your records before the effective date: 

  • Click the contact us button at the top of the page and send us an email requesting records. Please include all children with dates of birth. We will email all records to you directly. 

To request after April 30th:

  • Submit a records release with Morgan Records Management at: www.Morgan Records Management.com >Request my Records> Select your State & Practice> Complete Required Fields

If you have requested to have records faxed to a new physician, there may be a "Record Transer Fee." 

We are officially closed.
If you have an outstanding bill, you may pay this by selecting "Online Bill Pay" on this web page.  After June 30, 2026, outstanding balances will be sent to collections.
